2.0 Course Description

 

Students will progress through a step-by-step, practical application of Failure Mode, Effects, and Criticality Analysis (FMECA) in a dynamic instructor-led case study. This course details the best practice methodologies for: identifying critical product or system components, service attributes, and/or process steps; isolating specific potential failure modes and associated consequences; analyzing the root cause(s) for failure modes; prioritizing failure modes using multiple criteria (Severity, Occurrence, and Detection), and; developing and managing failure mitigation and action plans. We will also discuss data management methods required to support ongoing FMECA assessments.

 

3.0 Course Objectives

 

At the completion of this course, the student will be able to:

 

  1. Facilitate the development of a product, service, system, and/or process FMECA.

  2. Define quantified risk levels and designate effective mitigation actions.

  3. Discuss and describe Severity, Occurrence, and Detection ratings along with the summary score, the Risk Priority Number (RPN).

  4. Create and manage failure mode/risk reducing action plans.

 

4.0 Required Texts, Equipment, and Software

 

4.1 Books: Students will be provided with a printable Student Manual from Roxtar Consulting.

 

4.2 Equipment: Students are required to have access to a computer with internet access.

 

4.3 Software: Students are required to possess the following software packages and have them successfully installed and tested on their computers prior to the first day of class.

 

  • Microsoft Excel 2013 or newer

    • Roxtar Consulting provides each student with a Microsoft Excel FMECA template

  • Zoom (free version): https://zoom.us/signup

 

5.0 Certification Requirements

 

  • None – a course completion certificate is issued at the successful completion of this class.

 

6.0 Class Policies and Procedures

 

  • Courtesy is expected of all students.

  • Students are encouraged to participate in class discussions and activities.

  • You are expected to attend EVERY class.

  • Any unexcused absence will result in failed certification.

  • Be professional at all times. Students should come to class with the assigned reading and/or homework completed.

  • Makeup work will not be offered.

  • Late assignments, projects, etc. will not be accepted.
